How to Submit a TPIA Request

All Texas Public Information Act requests must be submitted in writing in one of the following ways:

1) email to TPIA@us.hctx.net.

2) fax to 713.437.4028.

3) mail to HCUS / TPIA Request / 406 Caroline St./ Houston, TX 77002.

Please include your name, phone number and email, and as much detail as possible to help fulfill the request.
